Activate Sports Therapy — Data Deletion Instructions

To request deletion of information associated with your Facebook login to Activate Sports Therapy, email Garth@activatesportstherapy.co.uk.

Please include:

• The name and email address used for your booking account.
• Whether you want to remove your Facebook connection, delete your booking account, or request deletion of other personal information.

You may use the subject “Facebook data deletion request”, although no particular wording is required. Please do not send passwords or medical information.

We may need to verify your identity before processing the request. We will remove the Facebook connection and associated Facebook login data, subject to any applicable lawful retention requirements.

Booking, payment and treatment records are assessed separately. Where particular records must be retained, we will explain why and the applicable retention arrangements.

We normally respond within one calendar month. If a permitted extension is needed, we will let you know.
